- Permit to Study from the head of your agency, if employed (OU Form 3)
- Certified True Copy of Official Transcript of Records/Transcript of Records with Remarks “Issued for BSU”
- Photocopy of PSA Birth Certificate
- Photocopy of PSA Marriage Certificate (if applicable).
- Payment of Application Fee of PhP 100.00 for Local Students and US $25 for Foreign Nationals/OFW (non-refundable).
- An essay indicating your purpose for applying to the Open University (at least 300 words with your name and signature below).
- Four copies of your latest identical ID Picture (2″ x 2″) with plain white background.
To follow (to be submitted during the enrollment period):
