Requirements: Valid I.D. Card

1. Diagnostic and Preventive Services
   
   1. Oral Examination / consultation
   2. Pit and fissure sealant
   3. Oral hygiene instruction

2. Curative Services
   
   1. Oral prophylaxis(scaling and polishing)
   2. Limited tooth restoration (filling)
   3. Extraction of unsavable tooth

3. Promotive Services
   
   1. Oral health lecture
   2. Display and distribution of IEC materials

4. Emergency dental treatment
5. Referral Services
6. Issuance of Dental Certificate

*For College students
            – Oral examination, consultation, or emergency dental care is from Mondays to Fridays. Treatment (oral prophylaxis, filling, or extraction) is on Mondays, Tuesdays, Thursdays, and Fridays. Treatment is done by appointment.

*For Employees and their immediate dependents
            – Treatment is done by appointment every Wednesdays.

*For BSU-Elementary Laboratory School and BSU-Secondary Laboratory School students
            – Dental services are oral examination, consultation, emergency dental care, oral health education and referrals only.
